To put a check mark in a box, place the cursor over the box and click. To remove the check mark repeat the process. You can also skip from box to box by using the TAB key and insert or remove  the check mark by using the ENTER key.

CAUTION: You cannot save the completed form if you are using the Acrobat Reader, so you will have to print as many copies as you think you will need, at least 2, one to sign and mail to WOW and one to keep for your records before you close the form. If you try to close the form a prompt will remind you of this and allow you to print before closing.
